moss growing on my soil

There is a green moss growing on top of the soil in some of my plants. What caused this and what can I do to get rid of it or prevent it?

Is it moss or algae?

..... add to Josh's questions: What are you using for soil and fertilizing with? Fish emulsion, by any chance?
