What in the World is doing this?

My poor plant made it through my college dorm life and now that I am in an apartment is struggling for dear life. Through my mistake he got fungus gnats--which I am taking care of right now through re-potting and yellow fly traps but something is eating his leafs.
Im not quite sure what type of plant I have...I'm pretty sure he is a small version of a mall plant but he has well over 20 leaves and I have no idea what to do.
I also seem to have attracted a green crane fly looking bug....but I don't know what it is because there is no reason a crane fly would be in my apartment that is no where near water

Let's see a decent picture of the plant so that it can be identified. The picture of the damage looks like a dog has chomped it. Can't you do better than that? (Not blurry) We love to solve mysteries.....and will, if given half a chance. :-)
