Help! Endless summer

I have and Endless Summer hydrangea, which is about 5 years old. Last year it was fine, but now this year it's struggling. There are only a few leaves, and they are only about 4'' long, coming from the base of the plant.

Any suggestions as to what I should do to help it along?

Where I am in zone 6, macrophyllas like Endless Summer were hit hard by the cold winter and died all the way back to the ground. Since polar vortex hit way harder in Northern Minnesota, your hydrangea probably had a near-death experience (unless you brought it inside for the winter?). If you think that's it, there's not much you can do but wait for it to recover, and make sure to protect it well this winter.

Both of my Endless Summer plants came through the winter fine. They are growing well and forming blossoms. I surround them with chicken wire and add leaves for the winter. Perhaps yours are just late.

Keep them watered and maybe give them some alfalfa tea or pellets/loose alfalfa.
