Is it the proper belt or is it a 'will fit'? Are the pulleys bent or misaligned? Are the idler linkages sloppy or bent? Is the belt properly routed?

The input shaft bearings on the transaxle are worn out
which is why you're throwing the belt. Grab the pulley on the transaxle and check for play. There are kits available to repair these, the transaxle will need to be removed however.
