WANTED: baby jade

HI
I TRANSPLANTED MY PLANT WITH SPAGHNUM PEAT MOSS. ITS IN A CLAY POT AND I WATER IT WITH A MOISTURE METER. MY LEAVES ARE ALL FALLING OFF AND THE STEMS ARE MUSHY. I HAVE IT IN A SOUTH WINDOW SO IT GETS SUN.
CAN YOU TELL ME WHAT I AM DOING WRONG THANK OU

Assuming this is a Crassula ovata (plant commonly called "jade plant"), It sounds like you're over-watering. Jades are succulents, which basically means they store water, and generally prefer being dry and only watered every two weeks or so.

Let the plant sit in gritty, quick-draining soil for a week or two and roots should start to develop.
