Transplanting seedlings

What is the best way to transplant a seedling into the garden?

Ae you supposed to loosen the rootball and surrounding soil and spread out the roots?
Or are you supposed to disturb the seedling and roots as little as possible when you transplant?

I've seen gardeners do this both ways. Or is it plant specific?

I recently transplanted some cauliflower seedlings which had some tangled up roots. I loosened up the roots and tried to spread them out before I transplanted into the garden, but i might have injured some roots when breaking it apart. 2 days later the cauliflower is struggling and it might have something to do with possible root damage when I transplanted.
