So if grafting a pear tree, i remove and graft the flower buds?

If i was to graft a pear tree then would i cut the flower bud and then insert that into the root stock?

And if the rootstock is an inch thick, wont that look wierd when bud starts growing out the side?
Can the top cut on the rootstock cause any infection?
Although in the picture the part that was grafted does not look like a flower bud.

Also, how will i know how big the tree will become if grafted on its own rootstock (pyrus calleryana)
