blueberry curling,browning, yellowing!! help!!

Symptoms: yellowing on leaves, curly green leaves, brown burnt looking holes in leaves.

Question: What's wrong with my blueberry bush?

Backstory:
I naively bought 2 blueberry plants couple years ago in hope that there wouldn't be much maintenance. The blueberries haven't produced much fruit since buying them. They also started to get sick this year (2014) in the early summer. I threw some irontone and coffee grounds on the top layer, nothing really helped. I lost one bush because all the leaves turned brown and started shedding off until there was nothing left.
Northern Kentucky soil is clay not allowing the roots to spread). The salvaged blueberry bush that I transplanted( in the picture) was put in a 15 gallon pot about 3 weeks ago. It's even got some new growth maybe as a result of liking its new home?

The composition of soil is pebbles at bottom, and about 60 percent peat moss, and 40 percent pine fines.

I've also been applying neem oil weekly in hopes that it will pests or fungus that is making my blueblerry bush sick.

Nothings helping..

Everyday I pull of burnt brown holed sick leaves..

Hey gatewoodgarden I'm in NKY as well. When you water your potted blueberry, how quickly does the water start running out the bottom of the pot?

I don't usually soak it long enough to see or hear water running out of the pot. It gets like half or less than half a gallon every other day or when the soil looks and feels slightly dry. The peatmoss that is mixed in with the pine fines holds alot of water.

Update- it has huge new leaves that are light dull green. I've noticed the disease is still present, I still don't know how to get rid of the disease. I'm also noticing flying lil light grayish blue bugs on the surface of the soil of my blueberry bush and my small bay tree-- they both have peatmoss in the soil, maybe that has something to do with it.
