Looks like Plum Curculio damage. On pear trees the damage is a hard dimple area where the half moon bite occurs. I bag my pears soon after blooming to reduce this damage. Chemical sprays are also available.

The tree is probably 20-25 feet tall and produces hundreds of pears, I don't think bagging really is an option for me. I have noticed some holes in the bark on the trunk as well, but I don't see that the plum curculio does this.

Or stink bug or other plant bug. Curc doesn't like pears so much. Not stoney pit though.

It looks more like stinkbug damage to me. I have a lot of pears looking like that now.

I guess you can rule out Stony Pit virus if the tree produced normal fruit in the past and hasn't been grafted since then. Else, how do you know it isn't Stony Pit until you cut it open an check the flesh? If it is Stony Pit it should have gritty brown spots, shouldn't it?

Stink bug injury can create the same thing.

Lack of complete pollination will also caused misshapen fruit, very noticeable on apples.
