Black Rot on Apple Tree?

I admit that this is a problem I've been ignoring, and now I'm afraid it's progressed too far to address it. I covet your advice to hopefully fix the bigger problem created by my procrastination.

As background, I have a 2 year old Granny Smith semi-dwarf that produced its first apple this year. This is my first time growing apples. The tree (leaves only) has been mildly affected by cedar apple rust disease although I've regularly sprayed with Immunox.

Is the black spot on my apple black rot? I also have a lesion on the main leader branch that I'm afraid should've been treated earlier.

What can I do to fix my tree?

This is the lesion on my tree. I think the fuzz might be a fungus, but I have no idea. Diagnosis is much appreciated.

Thats a standard summer rot, looks like a bird pecked it and the rot started from there. Theres a lot more rot this year than usual here in the mid-atlantic, all the late warm weather produced lots of it.

The stem looks like it could have been damaged by fireblight.

The white stuff is wooly apple aphid. Its usually not a big problem, just rub it off with your fingers or brush.

I lost most of my apples due to my ignorance/ineffective defense of summer rot. When is the effective timing to target this? I do have Captan, but sprayed for it after dumbly staring at it in disbelief for 2 weeks... Thought I was a genius after timing CAR, beating OFM/PC and deer this year. Got spanked again.
