What's the matter with my Apple Tree?

The leaves on my apple tree keep rotting, and I am seeing some worm-looking things at the base of the tree and at places up the trunk.

Are these roots? Should I be putting more soil against the tree? And about these leaves? Should I spray the tree with something? What?

This is a Home Depot 4 in one apple tree (Dwarf?) that I bought last season on sale. I only know that it is part braeburn (not sure what the other varieties are). I planted it in the spring of 2013 and this year it was full of apples that were eaten by wildlife before I could get any.

The things on the base of the tree are aerial burr knots, the rootstock is trying to sprout roots above ground. It sounds like the tree was grafted high. They don't really hurt anything other than giving bugs a place to hide. You could mound up dirt against the trunk, but I don't know how high they go.

The leaf problem could be an insect like leaf miners or caterpillars instead of disease.

The leaves look more like insect damage than rot. This late in the yr don't be concerned with that. There are leaf issues you might need to be concerned about next yr but mostly it's fruit issues and wildlife that are paramount.

The upper picture is indeed roots clustered in burr knots. I'd ignore them rather than piling up soil.

haykes...could you post a photo just a few feet from the tree...one that shows from soil level up to say, 2 feet above soil line?

Those leaves look just what I see with Jap beetle damage.
Do you have jap beetles in your area?

One key on jap beetle damage is that they seem to prefer
the younger leaves at the newest growth of apple and grapes. The older leaves from early in the season are usually left alone.

It could also be some other chewing insect but just commenting that it reminds me of Jap beetle damage.
