What do I do About My Lawn Now??

Well, I finally had sod put in, but, unfortunately, it has started to die in some parts (please see my pic). Here's what happened:

1) Sod was rolled out and watered 2x/day for 3 weeks.
2) Lawn grew to about 4-6 inches high (looked great), and was ready to be mowed.
3) I mowed the lawn and, I believe, the lawn may have been a little too high and also a little wet.
4) After I mowed, the lawnmower left clumps of grass, especially as the wheels rolled over the lawn (pic).
5) I let the lawn dry out for a day, then raked up as many of the lawn clumps as possible.
6) Continued to water for a few days.
7) Added fertilizer. Watered lawn thoroughly afterwards.
7) Continued watering 1-2/day.

Now the lawn is dead in some parts because of the clumps. How can I bring it back to a full, luscious lawn??
