Planting grass seed

About two years ago I decided to do something about that bare slope in front of my house. I brought in top soil, planted seed, fertilized it covered the area with straw. I watered, and we had about one half to and inch of rain per week for the next couple of months. I had great germination in a couple of weeks, and going into the winter had a great looking new yard. By the end of the following summer there was not one piece of grass on the slope.

I have done nothing for since then. The slope is now, Green, with a mixture of native NC wood grass and a little plant with flowers that dies back in the early fall.

I am now thinking about redoing the slope, and trying to determine what went wrong. One of the things that I now question is the state of the soil. As I said I brought in topsoil, spread it, and planted the grass with no attempt to let the new dirt settle.

Could the dirt have been TOO loose to sustain grass?
