Strange spike

I've had a phal for about three years. Nice blooms every year (42 at once spring '13). Now it has two 'normal' spikes that emanated from between the second and third levels down; their buds seem to be developing normally.

However, from the center of the top of the plant a month or two ago, it sent up a 'stem.' It is an inch long with a single leaf (about 6 inches) with another 'stem' growing out of the new center. That stem is over 5 inches long, has a little leaf developing, with a 'green mitten' growing from its center.

So what's going on? Is this normal?
