You don't mention what type of orchid you have. I'm assuming its a Phalaenopsis. Your orchid is dying because you are keeping it too wet and dark. Get it out of the container (not sure what you meant by 'bcontainer', but this plant will not grow under those conditions.

Put it in a pot with some bark and stick it in a window. It will grow if its not too late. You need to do some research on your plant. You are giving it the worse possible conditions.

Yes, it's a Phalaenopsis. When I first got it, it was jammed in a ridiculous small container. I immediately took it out since it was kept too wet all the time.
after taken it out of the small container I put it in a bigger container, I assume the roots tried to grow all over trying to fill it. The container was not translucent, so i changed it to another clear container. some of the roots just look dry and black here are some photos. Maybe someone can help identify what happened to it. it was also growing a new leaf but it stopped growing and it looks like it started to grow again. It's just the roots that don't look right to me. i water it once a week or when the moss is dry , it doesnt stay too wet since i check on it every few days.

Any roots exposed to the air will be dry. Any roots under the medium will be damp. You can't say the roots are dry by what you see on the top of the medium. Get a shishkabob skewer and put it down into the medium. When you think the plant needs water pull it out and touch it to the back of your hand. If it's dry or nearly dry, water. If it's wet, don't water. Phals don't like to dry out completely, so the skewer idealy should be damp. How wet or dry the skewer is before you water will depend on the needs of the type of orchid.
