ice damaging pond liner?

I had a very strange occurrence last winter.As winter approached and the temperatures dipped below freezing I put in my pond heater to keep pond from freezing solid as I did the past two years. All was well until we had a very cold spell,the ice was getting thicker and thicker but the heater kept a hole open in the ice as it was supposed to. The ice was starting to heave upward and kind of bow in the middle.One day I looked out to discover that the heater had slipped down under the ice and was floating in the water below. All I could see was a hole on top of the ice but no heater only the cord was showing. I had no idea what was going on. The water level started to drop below the ice and pretty soon I thought that the fish were not going to have enough water to survive. There was only about 10 inches of water and it was starting to freeze under the first layer of ice. I decided to fill the pond with water to bring it back to the top. The water began to drain down again after a couple of days. The fish survived and the ice eventually thawed but the pond never stayed full after that. It very slowly drained down about 6 inches over the course of a week. I had to replace the water all summer long. I plan to put in a new liner next spring. But I still would like to know what caused this. Was it the ice? Id hate to go through the expense of a new liner if this could happen again. Im hoping someone can shed some light on this for me.

We have let our pond freeze without a heater and the fish have not been effected. The freezing has not damaged the liner.

Could the heater have someway either melted the liner or loosen the glue in a joint that created a leak?

What kind of liner is in the pond? What is the size of the pond? What is the depth in the center? Are there shelves in the pond? If so what are the depths and widths? Is there rock on the bottom? Is there rock in the water at the waters edge? If so what kind of rock is it? Does it have sharp edges? What kind of heater was in the pond? Any water moving during the winter? Anything else in the water over winter?
