Has anyone run into problems with mold coming up through stained concrete floors?

Are you sure it is mold, and not mineral deposits from poorly cured concfrete or mineral-loaded groundwater?

It appears as a cloudiness beneath the clear coat sealer. We thought it was mold but it could be the concrete. Does concrete continue to react after it is poured?

Yes, concrete continues to harden for YEARS, and efflorescence (minerals appearing on the surface) can happen any time the slab is over wet soil.
