AGCO 23 HP riding mower

This rider was driven into the barn and stored, while the owner went away for a week. When he came home and wanted to use it, the belt between the engine pulley and the hydro had come off the engine pulley.
I put the belt back on. About 6 weeks goes by and the owner is using it normally and again drives it into the barn, while he went on vacation again. He went to use it and the belt was off the engine pulley again.
This particular model has stamped and bent sheet metal acting as belt guides. They are bent from the actual engine mounting deck and are part of the deck. The gap was so tight that I had to bend the tab outward to get the belt past the narrow clearance the first time. No amount of turning the engine and using a screwdriver would get the belt on the pulley before I bent it open.
I have never seen anything like this over the past 10 years. What could it be?
