boxed vs. B & B

Seems that at times, very large trees are sold "boxed". However, most of what I read says it's more common in the West and/or drier areas of the world. Around here, I rarely see boxed trees, except sometimes Japanese maples.

What are the pros/cons of boxed vs. B&B?

Are boxed trees dug and placed or do they actually grow them in the wooden boxes?

Do you remove the box bottom when planting?
