What's your take,.. should anything being done?
Looks like a bald faced hornet feeding on it.

Those fluffy things are probably woolly oak aphids. I'm not so sure about the scale. The shape doesn't quite suit me.

The fluffy things you see is snow. Just wanted to show what they look like in different stages.

Twig gall, probably caused by wasps. (link below) photos of this at the bottom of that page.

Thank you Dax for looking into,... but you haven't sold me on this, Scale looks closer to my I.D.

Yes, some kind of Insect is in these. There are approximately 30 species of kermes that occur in North America, but identification is difficult,..see link,...
